Documents Needed to File

Please compile all of the documents that apply to you. We will need them in order to properly file your bankruptcy.

	Documents Regarding Income
1	Paycheck stubs for you and your spouse, if married, for the last seven months
2	Documents showing commission or bonuses received during the last seven months
3	Documents showing unemployment benefits received during the last seven months
4	Documents showing child or spousal support paid or received during the last seven months
5	Documents showing pension or retirement benefits received in the last seven months
6	Documents showing bank interest or stock dividend income received during the last seven months
7	Documents showing rental or lease income received during the last seven months
8	A list of all money you received from anyone else to help cover the living expenses of you or other members of your household
9	Bank statements for the past seven months for all bank accounts

	Documents Regarding Your Debts
10	All bills, demand letters and or collections notices received in the last 90 days
11	All lawsuits or garnishment documents received in the last year
12	A copy of your credit report can be obtained for free from www.annualcreditreport.com or any of the three major reporting agencies
13	A current mortgage statement showing outstanding balance and any existing arrearages
14	Most recent property tax statement

	Documents Regarding Your Vehicles
15	Retail Installment Contracts for any vehicle you owe money on
16	Proof of insurance for any motor vehicle that you owe money on
17	Copies of all vehicle registrations or titles

	Miscellaneous Documents
18	A copy of your (and your spouse's) drivers license and social security card
19	A copy of your federal and state income tax returns for the last two years (if filed)
20	A copy of your most recent statement for any 401(k) or pension
